Advanced Practice Provider

We are seeking a highly skilled and experienced Advanced Practice Provider to join our dynamic healthcare team. As an Advanced Practice Provider, you will play a pivotal role in delivering high-quality care to our patients, providing comprehensive medical services in collaboration with our physician team. This position requires expertise in assessing, diagnosing, and treating a wide range of acute and chronic conditions. The ideal candidate will have exceptional clinical knowledge, excellent communication skills, and a passion for providing exceptional patient care.

Responsibilities

• Conduct comprehensive patient assessments, including medical history, physical examinations, and diagnostic tests
• Develop and implement individualized treatment plans for patients, based on their specific needs and medical conditions
• Prescribe and administer medications, treatments, and therapies, in accordance with established protocols and guidelines
• Monitor patients' progress and response to treatment, adjusting the care plan as necessary
• Educate patients and their families on disease prevention, management, and lifestyle modifications
• Collaborate with interdisciplinary healthcare team members to ensure coordinated and efficient care delivery
• Maintain accurate and thorough documentation of patient encounters, treatments, and progress

Requirements

• Master's degree in Advanced Practice Nursing or Physician Assistant Studies
• Current certification and licensure as a Nurse Practitioner or Physician Assistant
• Minimum of [x] years of experience as an Advanced Practice Provider in a clinical setting
• Proficiency in conducting physical exams, diagnosing conditions, and formulating treatment plans
